Our Office Policies

We look forward to seeing you on your scheduled appointment date. If you are a new patient, pre-registration forms are now available to download on our website in an effort to streamline the check-in process. We will contact you a few days prior to your appointment to confirm the appointment date and time. To pre-register you may also go to the Patients Portal. Upon arrival at our office for your appointment, we will need to copy your insurance card(s) in order to bill your insurance company and enter you registration information.

Appointments/Cancellations

In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment. If you are unable to keep your appointment and do not provide 24 hours notice or do not show for your appointment there is a fee that will be applied. This fee is not reimbursed by insurance carriers.

Emergencies

We prefer to see major emergencies in the Emergency Room at Bridgeport Hospital so that procedures, such as scans and blood-work, can be done immediately and appropriate emergency care can be provided. Emergency after-hours coverage is provided by the On-Call doctor, who can be reached by dialing the main number: 203-377-5988.

Prescription Medicine Refills

Prescriptions will be refilled only during office hours. No refill requests will be taken by the answering service.

Patient charts are maintained in Stratford (including Fairfield charts) and Shelton and access to this information is essential in medication refills.

OFFICE POLICY ON PRESCRIPTIONS

Call your pharmacy and they will send a request for refill electronically or by fax.

Due to the high volume of prescription requests 72 hours will be necessary in order to process requests. Requests for refills may require a follow up appointment with your doctor. When requesting a written medication, please be prepared to provide your name (spelling your last name), date of birth and the name of the prescription and allow 72 hours and you may pick it up at the office you are seen at. For mail order requests please call them and have them electronically request the refill or bring in the form that they require and allow 72 hours for us to process your request. Please monitor your medications carefully so that you won't run out.

Payment

For your convenience, we accept cash, check, Visa, Master Card. Any copay or deductible is due at time of appointment. To set up a payment plan you may contact Billing: 203-690-5100.

Patient Forms

Neurological Specialists, P.C. provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
